Innovative Wound Solutions, a leading healthcare organization dedicated to providing innovative and patient-centered care, is seeking a highly motivated and experienced Nurse Practitioner to work within our established Mobile Wound Care Program. We are committed to delivering high-quality, personalized care to individuals with complex wounds, and we believe in empowering our healthcare professionals to make a significant impact on patient outcomes.

Position Overview:
We are looking for a dynamic and autonomous Nurse Practitioner to serve the Verde Valley market. The successful candidate will play a crucial role in providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes.
 
Responsibilities:
  • Clinical Leadership:
    • Provide direct patient care through all available channels and products.
    • Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices.
    • Monitor and evaluate patient progress, adjusting treatment plans as needed.
    • Collaborate with interdisciplinary teams to ensure seamless patient care coordination.
  • Chronic Wound Care Planning and Management:
    • Develop personalized treatment plans for patients with wounds, focusing on long-term management and prevention of complications.
    • Educate patients and families on self-management strategies and lifestyle modifications. 
  • Communication and Collaboration:
    • Establish and maintain effective communication channels with referring providers to ensure continuity of care.
    • Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care.
 
Qualifications:
  • Current and unrestricted Nurse Practitioner license in Arizona.
  • Certification as a Nurse Practitioner with a specialization in wound care management is preferred.
  • Minimum of 1 year of experience in wound care.
  • Strong interpersonal and communication skills.
